GUIDELINES FOR CERTIFICATE OF GOOD MORAL CHARACTER REQUEST:

1. Your request for a Certificate of Good Moral Character ( hard copy) will be processed once payment is made.  The processing fee for One (1) copy of Certificate of Good Moral Character is Php 100.00.
   Payment may be made thru the following options:
   1. You may pay the school cashier - please secure a payment notice form from the Guidance Office and proceed to the Accounting Office for payment
   2. Or you may deposit/ transfer funds to:

2. Upload the payment confirmation/ reciept in the request form.
3. A notification email will be sent for the release and pick up details. 
4. Please allow at least Two (2) to Five (5) processing days after request is made.
5. Kindly make sure to fill out ALL REQUIRED fields in the request form below.  A notification will appear once you have successfully submitted your request.
