GUIDELINES FOR CERTIFICATE OF GOOD MORAL CHARACTER REQUEST:

1. Your request for a Certificate of Good Moral Character (electronic copy or hard copy) will be processed once payment is made.  The processing fee for One (1) copy of Certificate of Good Moral Character is Php 100.00.
   You may deposit or transfer funds to:

2. Electronic copy requests of Certificate of Good Moral Character will be sent to your email address after at least Two (2) to Five (5) working days after request is made.
3. For Hard Copy requests, a notification email will be sent for the release and pick up details.
4. Kindly make sure to fill out ALL REQUIRED fields in the request form below.  A notification will appear once you have successfully submitted your request.
